MetaForm is now live! (1) Create new forms
Hello mate, we are MetaCRM — a group of builders wanting to make Web3 CRM better by providing Blockchain-native one-stop CRM solution!
Today we would like to introduce our first product — MetaForm.
MetaForm is the first Blockchain-native survey tool to complete all tasks at once. MetaForm is a Blockchain-native survey tool to collect both off-chain information & on-chain data with one-stop “precise airdrop” feature.

MetaForm Tutorial
(1) Create a new form
To create a new form, click Create New as shown in below screenshot
(2) Enter basic information about the form
- Give it a name: Type a proper name for your new form
- Date Setting: Select a specific period to open your form
- Cap of survey: Limit the number of forms you want to receive
- Restriction: Set up restrictions to precisely filter your surveyees (e.g. Hold specific NFTs; hold more than certain amount of ERC20 tokens)
- Reward: Choose rewards you would like to distribute to surveyees; currently support ERC20 & ERC721
(3) Add your questions
- Content: In this section, you can browse all the questions you’ve created. To create a new question, press “add” on the top right of this section.
- Type: MetaForm has various types of questions. Creators can select the most suitable question type for their questions.
- Background: Creators could upload an image for each question; it could be shown as a demo picture or as the background
- Ending Page: Don’t forget to leave a thank you message for your surveyees at the end
(4) Share your form
Always preview your form before publishing for the sanity check. Once the preview is completed, simply click the “publish” bottom and share the link with your potential surveyees
(5) View the form
After you publish your form, you can edit, preview, share, check response results, or delete the form in Workspace. If you want to pause the survey for a while, you could also switch the form’s status between activate & deactivate.
